In the first part of this talk, I will present some insights from experiments performed with patients with refractory epilepsy under intracranial EEG (i.e., Electrocorticography-ECoG) monitoring. Specifically, our findings demonstrate that language comprehension involves not only auditory regions but also engages sensorimotor areas with similar temporal signatures. The activation of auditory and sensorimotor areas is accompanied by parallel suppression of visual and other non-task-relevant areas during syllable perception. The invasive functional characterization of the cortex during ECoG is mainly aimed at increasing the patients’ chances of becoming free of seizures. Therefore, in the second part, I will present a new method for the identification of eloquent cortical areas from cortical responses by using low-frequency activity power modulation. This method helps in the mapping procedure by allowing the optimization in the planning of the electrical stimulation mapping. Finally, I will conclude with challenges and possible solutions that our group has faced in carrying out ECoG-based studies.
